Use this request to submit your check-in documentation as a New International Student and every time you leave and re-enter the U.S. Please do not submit this request until after you have entered the United States.

Use this request if you are a current international student at Earlham. You must complete this request before the beginning of each semester. CGCE must register your SEVIS record within the first 30 days of the semester, per F-1 visa regulations. This request must be completed each semester that you are pursuing your academic program. 

Use this request if you are an F-1 student at Earlham who needs a letter proving your immigration status.

If you are applying for a Social Security Number, please use this request to have a letter issued from CGCE .

If you live on campus, use this request for a Proof of Address letter for applications to BMV; such as a Indiana State ID, Driver’s License, etc.

Use this request if you will not complete your program by your Program End Date. Please note you must request an extension at least 2 weeks before your Program End Date.

If your I-20 has been damaged, lost or stolen, please use this request for a reprinted I-20.

Use this request if you need to apply for an Emergency Loan based on financial hardship.

Use this request in order to receive an updated I-20 for returning from a Leave of Absence (LOA) or Suspension.

Use this request to apply for Curricular Practical Training (CPT). Please note you may not begin working until your CPT has been approved by CGCE and you have your CPT I-20 in hand.

Use this request to apply for Optional Practical Training (OPT).

If you will complete your Earlham undergraduate program before the Program End Date on your I-20 (e.g. early graduation, transferring out for
3 + 2 Engineering program, starting Master’s portion of 3 +1 program, etc.), please complete this request.

Use this request to begin applying for the 24 Month STEM Optional Practical Training (OPT) Extension benefit. Please note that you cannot apply for STEM OPT more than 90 days before your current 12 month OPT end date.

Use this request to submit any changes in your employment or personal information to CGCE within 10 days of the change, per the F-1 visa regulations.

Use this request to submit your 6-month SEVIS Validations and Annual Self-Evaluations while on a 24-Month STEM OPT Extension, per F-1 visa regulations.

Use this request if you have received Cap-Gap as part a result of your employer filing an H-1B petition with USCIS
